When calculating a covered individual's overall contribution to any out-of-pocket maximum, deductible, copayment, coinsurance, or any cost-sharing requirement under a health plan, a covered entity or pharmacy benefits manager shall include any amounts paid by the covered individual or paid on behalf of covered individual by another person.
