Every contract or certificate form, or amendment thereof, including the rates charged therefor by the corporation shall be filed with the Commissioner or the Green Mountain Care Board established in V.S.A. chapter 220, as appropriate, for the Commissioner’s or the Board’s approval prior to issuance or use. See definition section Vt. Stat. Ann. tit. 8, § 4511.
