Every enrollee is entitled to an evidence of coverage issued by the health maintenance organization. No schedule of charges applicable to individual health maintenance organization contracts may be used until a copy of the schedule has been filed with and approved by the director or his designee. See definition section S.C. Code Ann. § 38-33-20.
