States that no benefit plan is subject to a law that inhibits insurers from selectively contracting with provider groups, imposes restrictions on the ability of the insurer to negotiate with providers regarding the level of reimbursement, requires the insurer to include specific providers or exclude specific providers, limits the financial incentives a plan may require a beneficiary to pay when a nonplan provider is used on a nonemergency basis, prohibits utilization review, requires use of specified standards or requires the disclosure of specific criteria, among others.
