States that no payer shall deny reimbursement to a hospital or physician for covered services rendered to covered persns on the grounds of medical necesity in the absense of fraud or misrepresentation if the hospital or physician requested authorization and received approval or the payer didn’t respond, or the patient is no longer eligible to receive coverage from the payer.
