States that any health insurer that has a plan that qualifies as a high deductible health plan with health savings account,shall provide biannual surveys to the Department of Banking and Insurance based upon requested information from subscribers, including information concerning: income levels, type of contract which previously provided coverage to those individuals, out of pocket expenses and percent of income used to pay the deductibles
