There is a Community Integrated Medical Home Program, whose mission is to keep Maryland families healthy through the use of innovative mapping tools that allow better targeting of resources to those in need; coordinate comprehensive services provided by a participating patient centered medical home with public health resources in local communities across the State; and provide complementary support for qualified individuals between office visits.
