Each application for enrollment in an HMO must state that the enrollee will personally bear all costs if they utilize health care not authorized by the plan. Every enrollee at the time of enrollment and annually thereafter shall receive a written notice which fully explains copayment and deduction amounts applicable to each covered service. This shall also outline limitations on choice of primary care providers, access to specialists, and application of preexisting condition exclusions. See definition section La. Rev. Stat.
