Statutes require HMOs to provide to the subscriber a list of providers who provide health care services through the HMO. HMOs must provide to each enrollee information on how services can be obtained and costs for which the enrollee or subscriber is responsible. See definition section Ind. Code §§ 27-13-1-1 through 27-13-1-34.
