A limited service health maintenance organization is subject to a number of applicable statutes, including Ind. Code § 27-13-15-1(a)(2) concerning gag clauses in contracts and Ind. Code § 27-13-36-5 concerning out of network providers. See definition section Ind. Code §§ 27-13-34-1 through 27-13-34-6.
