Statutes require that all applications for insurance and schedules of premium rates be filed with the commissioner. The commissioner is required to disapprove any such form if the benefits provided therein are not reasonable in relation to the premium charge. An insurer may revise its schedules of rates from time to time, but such revised schedules need to be filed with the commissioner.
