Insurance; health insurers; amount of co-pay for primary care physician visits; limit. Amends 1956 PA 218 (MCL 500.100 – 500.8302) by adding sec. 3406u. A health insurance policy delivered, issued for delivery, or renewed in this state that provides coverage for a primary care provider visit must not require an insured to pay more than a $5.00 co-pay per visit.
