When a health insurer sends an explanation of benefits paid statements or claims summary statements to an insured it shall be in a format and written in a manner that promotes understanding by the insured by setting forth any reductions in the amount paid due to co-payments or deductibles, any reduction in the amount due to the application of any other policy limitation or exclusion and the total dollar amount paid and what is remaining unpaid.
