A subscription agreement in a form approved by the department of public health shall be issued to each subscribing member of a medical service corporation. In addition to any other requirements such a form must meet the minimum readability standards. Such agreement shall include a full and accurate statement of the benefits of membership, the medical services excluded if any, and the terms of duration, cancellation and termination.
