An Act To Create A Pilot Program To Be Known As The Medicaid Health Homes High Utilizers Pilot Program To Identify And Engage Medicaid Beneficiaries Who Are At Greatest Risk For High Utilization Of Hospital Or Other Health Home Services; To Provide That The Pilot Program Shall Serve Targeted Medicaid Beneficiaries In Hinds, Madison And Rankin Counties, And Shall Be Administered By The Division Of Medicaid; To Specify The Minimum Components Of The Pilot Program
