Utah Code § 31A-22-640. Insurer and pharmacy benefit management services–Registration–Maximum allowable cost–Audit restrictions: Accident and Health Insurance – Utah

A pharmacy benefit manager shall not use maximum allowable cost as a basis for reimbursement to a pharmacy unless the drug is listed as “A” or “B” rated in the most recent version of the Orange Book and the drug is generally available for purchase in this state from a national or regional wholesaler; and not obsolete.
