Sets forth requirements for preferred provider plans or policies, including that benefit levels for health care services provided by nonpreferred providers shall be at least 80% of the benefit levels for services rendered by preferred providers, and that, in the case of emergency care from a nonpreferred provider, payments be made at the same level and in the same manner as if the covered person had been treated by a preferred provider.
