AN ACT RELATING TO STATE AFFAIRS AND GOVERNMENT — THE RHODE ISLAND HEALTH CARE REFORM ACT OF 2004–HEALTH INSURANCE OVERSIGHT: Requires the health insurance commissioner to adopt a uniform set of medical criteria for prior authorization and create a required form to be used by a health insurer.
