Statutes prohibit an insurer or administrator from refusing to contract with any noninstitutional provider who meets the terms and conditions established by the insurer. Statutes allow an insurer to issue or administer programs or policies that include incentives for the insured or beneficiary to utilize the services of a provider which has entered into an agreement with the insurer. Statute requires administrators of a preferred provider program to register with the Department of Insurance. Any preferred provider organization providing hospital, medical or dental sercices must include a program of utilization review.
